There will be no on-site registration.  Please return the registration form in a timely manner prior to October 19, 2011. The form will be available at www.ihsaa.org after June 1, 2011.

For attendance at the clinic to count you must:                                                                                                                                                                               

1. Sign in before the time indicated.                                                                                                                                                                                                     

2. Attend all 4 hours of instruction and actively participate.                                                                                                                                                      

3. Pay the $2.00 registration fee at the door.                                                                                                                                                                                     

4. Complete and return the attendance form to the clinician at the end of the session.
